After completing MBBS from a foreign university, Indian students must pass a licensing examination to practice medicine in India. Currently, this is the Foreign Medical Graduate Examination (FMGE), which is being replaced by the National Exit Test (NExT). This comprehensive guide covers everything you need to know about both exams.
Finish 54-month MBBS + 12-month internship at the same foreign university (per NMC FMGL 2021).
Submit documents to NMC including degree certificate, transcripts, NEET scorecard, and passport.
Pass the licensing exam. Currently FMGE (single MCQ test); transitioning to NExT (theory + practical).
After passing, complete a 12-month supervised internship at an NMC-recognized Indian hospital.
Register with State Medical Council and NMC. You can now legally practice medicine in India.

FMGE has historically had low pass rates, making proper preparation essential. Here's the trend:
| Year/Session | Total Candidates | Passed | Pass Rate |
|---|---|---|---|
| December 2024 | ~28,000 | ~6,200 | ~22% |
| June 2024 | ~26,500 | ~5,800 | ~21.8% |
| December 2023 | ~27,000 | ~5,500 | ~20.4% |
| June 2023 | ~25,000 | ~5,000 | ~20% |
The low pass rate is mainly because many students study in universities that don't follow Indian medical curriculum standards, or they don't prepare adequately. Students from NMC-compliant universities recommended by Medipedia have significantly higher pass rates — typically 50-70% — because our partner universities follow curricula aligned with Indian standards and we provide FMGE coaching support.
The National Exit Test (NExT) is a two-step examination that will replace FMGE for foreign graduates and also serve as the exit exam for Indian MBBS graduates. NExT scores will additionally be used for PG admission.
Begin FMGE preparation from 3rd year of MBBS. Study Indian reference books (Robbins, Guyton, Harper) alongside university curriculum. Medipedia provides online FMGE coaching from final year itself.
Medicine, Surgery, OBG, and Pediatrics carry the most weightage. Master these four subjects first. Pharmacology and Pathology are also crucial.
Practice at least 10 years of previous FMGE papers. Understand question patterns and commonly tested topics. Use apps like PrepLadder and Marrow.
Maximum clinical exposure during your MBBS abroad helps immensely. Actively participate in hospital postings and OPDs. Practical knowledge is essential for NExT Step 2.
Medipedia Overseas provides online FMGE coaching from the final year of MBBS. Our coaching partner has helped students achieve 60%+ pass rates — nearly 3x the national average. We believe in not just getting you admitted abroad, but ensuring you come back and succeed in India.
